Loose Pulley Motor Y Failure on Selftest

I purchased a Prusa i3 MK2S kit in December 2017 and have completed the assembly but it continues to fail the selftest displaying "loose pulley motor Y". I have verified that the set screw on the shaft of the motor is on the flat part and that it is not slipping. I've loosened the belt and I've tightened the belt. My heatbed seams to move fine when I run the XYZ calibration. I have seen this question asked repeatedly but have not found a solution that fixes my printer. What else can I try?

I've followed your instructions. I upgraded the firmware. I also held the Y motor shaft with pliers while pushing on the heatbed and I did not have any slippage. The heatbed didn't move until I pushed hard enough to lose grip of the motor shaft. I repeated this several times. Still getting the "loose pulley motor Y" on the self-test.

I ordered an 8 inch electronic caliper and checked all my measurements. There were two that were slightly off. The 100mm and the 178mm. I adjusted them and the self-test passed.
